Term for issuance of an international Ukrainian passport

The term for issuance of an international Ukrainian passport (excluding the terms of delivery to the Consular Section of the Embassy of Ukraine in the USA) is up to three months from the date of submission of documents. In the case of expedited delivery, the term of issuance passport is about 6-7 weeks.


The validity of a Ukrainian biometrical international passport

10 years - for persons who have reached 16 years old

4 years - for persons under 16 years.


Is it possible to issue an internal Ukrainian passport at the Embassy of Ukraine in the USA?

The Consular Section of the Embassy of Ukraine in the USA doesn`t take any actions regarding an internal Ukrainian passport. Regarding these actions, we recommend contacting the State Migration Service of Ukraine. 


Leaving an old passport on hand

The old passport can be left in case it has a valid visa or you need it to obtain appropriate permits from US authorities.

Attention! No more than two passports could be on hand including those that had expired.  


Issuing a passport with a new name

Applying for a new international passport in the case of a change of name in the Embassy could be made only if the applicant has a permit issued by the competent authorities of Ukraine to leave for permanent residence abroad. Appropriate stamps should verify this fact.

STEP 1. Collect the necessary documents according to the list (see the tab "Documents for issuing an international Ukrainian passport").

STEP 2. Make an online appointment for a consular reception at the Embassy of Ukraine in the USA  - https://online.mfa.gov.ua

IMPORTANTLY! A separate appointment should be made for each person!

WARNING! Consular reception is by appointment only. 

Documents should be submitted PERSONALLY. The consular department does not accept documents by mail.

Application for a foreign passport for children under 12 years of age can be made by parents or a legal representative in the absence of the child.

STEP 3. Submit documents in person.

If a person is over 16 years old, then the personal presence of this person is mandatory and the documents are submitted in person.

If a person is younger than 16 years, but older than 12 years, then the personal presence of this person is mandatory and the documents are submitted by one of the parents.

If the person is younger than 12 years, then the personal presence of this person is not mandatory and the documents are submitted by one of the parents.

DOCUMENT 1. Completed application form for the issuance of a foreign passport together with consent to the processing of personal data.

WARNING! If documents are submitted for the issuance of a foreign passport for a child under the age of 16, from the second parent, provided that he is a citizen of Ukraine, an application-consent for the issuance of a foreign passport for the child is submitted:

in the usual written form - if the other parent is personally present when submitting the documents;

notarized (in original) - if the second parent is absent when submitting the documents. If the application is certified by a US notary public, the notary's signature must be additionally apostilled.

If the second parent is not a citizen of Ukraine, his/her passport certifying his/her citizenship is submitted.

If the second parent is deprived of parental rights, a corresponding court decision is submitted, with a note that it has entered into legal force.

If the parents are divorced, a corresponding court decision (noting that it has entered into legal force) on the dissolution of the marriage is submitted, as well as a document in which it is determined that the child permanently lives with the parent who is applying for the child's foreign passport.

If the entry about the father is entered from the words of the mother (on the basis of Article 135 of the Family Code of Ukraine), a corresponding extract from the civil status registration act is submitted.

DOCUMENT 2. Old foreign passport (regardless of whether it is expired) + copies of 1-2 pages, as well as a copy of the page with records and marks (if such records are available) about enrollment of children, permanent residence abroad, consular record.

If the applicant was not documented by a foreign passport, but entered the US territory on the basis of the father's foreign passport with a pasted photo of the applicant, such a passport document is submitted (if it is not available - its copy).

If a passport document is issued for a child who has not reached the age of 16, the passports (foreign or domestic) of both parents are additionally submitted.

If the foreign passport was lost - a Police Report is submitted, which certifies the application of the applicant to the US police authorities in connection with its loss, together with the corresponding application. A copy of the lost passport is submitted if available.

If a passport is issued to a child who was born outside of Ukraine and was not documented by a passport document, a certificate of registration of the person as a citizen of Ukraine + its copy is submitted.

DOCUMENT 3. Internal passport of a citizen of Ukraine or birth certificate (if the applicant does not have the document in hand, a copy may be submitted) + its copy.

WARNING! If a different surname is indicated in the domestic passport or birth certificate than in the foreign passport, a MARRIAGE CERTIFICATE is additionally submitted. If the marriage was not concluded in Ukraine, the marriage certificate must be apostilled and translated into Ukrainian.

DOCUMENT 4. Document confirming the legal basis of residence in the USA + its copy.

It may be:

  • green card
  • form I-94 (if you are in the USA on the basis of a visa)
  • certificate from the place of work

DOCUMENT 5. Document with current address of residence in the USA + its copy

  • driver license
  • invoice for payment
  • apartment rental/purchase agreement

WARNING! If in DOCUMENT 4 or 5 a different surname is indicated than in the foreign passport, a document indicating the change of surname (marriage certificate, divorce certificate), apostilled and translated into Ukrainian is additionally submitted.

DOCUMENT 6. Money Order + its copy

If a foreign passport is issued to a person 16 years of age or older - Money Order for $130

If a foreign passport is issued to a person younger than 16 years - Money Order for $40

WARNING! The fields "Pay to" - "Embassy of Ukraine", "Pay from" - the name of the applicant must be filled in the Money Order.

Cash, credit cards, checks, bank transfers are not accepted!

A Money Order can be issued at your bank branch or via USPS.
