Documents should be submitted in person, by a legal representative (parents, guardians, adoptive parents) or by power of attorney.
If the power of attorney is certified by the U.S. or other country notary, it must be duly authenticated (Apostille/legalize), with a notarized Ukrainian translation, if it is written in a foreign language.

List of documents to submit to the Embassy:
- Filled in applications - anketa-request-documents.pdf in 3 copies. The application should be completed in Ukrainian, typed in block letters in black or blue ink, dated and signed by the applicant; ;
- Filled in application for Apostille - apostille-application.pdf (only if you need an Apostille certified document);
- Original valid passport and its copy;
- Confirmation of relations (original certificates), if needed.
- Consular fee:
a) For the document without Apostille: Money Order or Cashier Check payable to the Embassy of Ukraine for 20 dollars USA;
b) For the document with Apostille: Money Order or Cashier Check payable to the Embassy of Ukraine for 50 dollars USA along with receipt of bank transfer for 51 UAH (2-2.5 dollars USA) to the State Treasury Service of Ukraine (https://www.treasury.gov.ua/diyalnist/rekviziti-valyutnih-rahunkiv).
